2010-2011 Forms & Information

While you and your family are enjoying summer vacation, plans are under way for the start of the 2010-2011 school year. To facilitate a smooth opening of school, it is necessary that certain information be on file before the school year begins. All the forms listed below can be found in one file in the column to the right, or you may access each form individually via the links below. Upon request, the front office will mail families printed copies; simply call us at 904.396.5751.

Parents or legal guardians of all students must complete and return the necessary forms to the Front Office by August 2, unless otherwise noted, with all necessary signatures.  Required forms for 2010-2011 include:

  1. 2010-2011 Student Permission Form: This form consists of two specific authorizations; medical treatment and counseling in the event of a medical emergency and attendance and transportation for school-sponsored trips, etc. 
  2. 2010-2011 Student Photo, Media, Social Media and Video Release Form: This form consists of authorization to use photo, video or other media of students collected by the School or a third party in any School publication or communication.
  3. 2010-2011 Health Information Form: This form provides the School a way to respond quickly in the event of a student's illness.  It also provides a current record of your child’s medical information, including contact numbers for the parent or legal guardian. It also authorizes the School clinic to dispense or withhold specific medications.
  4. 2010-2011 Bullying, Hazing and Harassment Policy: This form is the affirmation of agreement to the School's policy on bullying, hazing and harassment.
  5. 2010-2011 EHSNET Acceptable-Use Policy: This form is the affirmation of agreement to the School's policy on acceptable-use of the EHSNET network.
  6. Parent/Student Directory Update Form: This form provides the School with current information for the Parent/Student Directory, which you may opt out of being listed in.  It also provides the School with current information for important communications throughout the year.
  7. Grandparent Information Form: This form puts grandparents on mailing lists for special events, including Grandparents’ Day as well as The Magazine of Episcopal.
  8. FHSAA Preparticipation Physical Evaluation (Form EL2): This form must be dated after June 1, 2010.
  9. FHSAA Consent and Release from Liability Certificate (Form EL3) : This form must be dated after June 1, 2010.

New Families Only

In addition to the forms on the left, new students must have the following forms on file in the Admissions Office by July 16 (forms are available from your student’s doctor):

  • State of Florida school entry health exam: DH3040 (dated on or after August 17, 2008)
  • Florida certification of immunization: DH680 (all immunizations must be documented)
