2009-2010 Forms & Information

While you and your family are enjoying summer vacation, plans are under way for the start of the 2009-2010 school year. To facilitate a smooth opening of school, it is necessary that certain information be on file. All the forms listed below can be found in the column to the right, or you may access each form individually via the links below. Upon request, the front office will mail families printed copies; simply call us at 904.396.5751.

Parents or legal guardians of all students must complete and return the necessary forms immediately. Required forms for 2009-2010 include:

  1. Grandparent information: this form puts grandparents on mailing lists for special events, including Grandparents’ Day.
  2. 2009-2010 Student Permission Form: the parent or legal guardian must complete and sign this form. It consists of three specific authorizations:
    a. medical treatment and counseling in the event of a medical emergency
    b. attendance and transportation for school-sponsored trips, etc.
    c. photo and video release
  3. Health Information Form: in the event of a student’s illness, the School will be able to respond quickly using the information you provide. A parent or legal guardian must sign this form. It provides a current record of your child’s medical information, including contact numbers for the parent or legal guardian. It also authorizes the School clinic to dispense or withhold specific medications. Note: Each Health Information Form must be signed before it is returned to the School.
  4. Sports Participation Forms: FHSAA Physical and Parent Consent forms (EL2 and EL3): these must be dated after June 1, 2009.
  5. Network Acceptable-Use Policy (AUP)
  6. Student Directory Update Form (due July 1). Note: family email addresses will be included in the printed directory this year. If you do not wish to have your email addresses printed, please be sure to check the “Do not publish email address” box.
  7. Bullying, Hazing and Harassment Policy

New Families Only

In addition to the forms on the left, new students must have the following forms on file in the Admissions Office by July 1 (forms are available from your child’s doctor):

  • State of Florida school entry health exam: DH3040 (dated on or after August 17, 2008)
  • Florida certification of immunization: DH680 (all immunizations must be documented)
